Certified that Mrs Daulannesa, aged about 83 yrs,w/o late Md. Shahabuddin now residing at her eldest son’s house at flat no-1/B, house no-513,road no-8(old-21),Mirpur DOHS, Dhaka-1216. She is suffering from multiple polyarthetics and severe ostropoges, causing her bed ridden and disabled. She is unable to move out of bed and passing a crippled life on her bed since a long period. She is under my treatment from June/2013.
